D’après leur avis de grève, « leur mouvement d’humeur fait suite à leur communiqué n°4 du 17 août 2021 relatif au retard du paiement des bourses de spécialité, ainsi que la réticence du MSAS (ministère de la Santé et de l’Action sociale) à s’engager dans un large chantier de réformes visant à améliorer le quotidien des spécialistes sur l’ensemble du territoire sénégalais, ce qui s’inscrit indéniablement dans le cadre d’une amélioration de l’ensemble du secteur sanitaire au Sénégal.”
En effet, ajoutait la note parcourue par Emedia, « le COMES déplore avec véhémence cette attitude des autorités qui consiste à repousser les échéances de la réalisation de ses promesses à des délais indéterminés ».

La preuve, rappelaient-ils dans une lettre ouverte adressée à la tutelle, déjà en avril dernier, « les médecins en spécialisation communément appelés DES sont titulaires d’un diplôme de Docteur d’État en médecine, à la suite de quoi ils embrassent une filière de spécialisation afin d’affiner leurs connaissances pour le grand bonheur de nos malades. Dans ce cadre, ils assurent des activités de consultation, le suivi des patients hospitalisés, les permanences, les gardes ainsi que certains actes médicaux et chirurgicaux, totalisant ainsi un volume horaire dépassant les 40 heures par semaine. Dans l’écrasante majorité des hôpitaux de la région de Dakar la continuité du service est assurée, pour l’essentiel (plus de 80%) par les DES. Dans plusieurs régions où il n’y a pas de spécialistes, les DES qui effectuent un stage rural après 4 à 5 années d’apprentissage, assurent le fonctionnement de leurs services respectifs
